As part of their mandate, the Health Authority is also the only state level agency that is authorized to issue birth records, and the agency keeps records of birth, and other vital records, as far back as the year 1903. Records of birth occurring that date may be found at the local issuing authority of the county where the birth occurred or at the state archives.
To request for a birth record at the Oregon Health Authority, the searcher should first download the relevant form from the website of the Health Authority and to complete the same. Note that the form must be notarized because the request is for a certified copy of a birth record. Do not forget to include a copy of any government issued identification card with the signature of the searcher at the form. The next step would be to make a money order payable to the Health Authority to correspond to the required fee. The required fee is twenty dollars, plus fifteen dollars for every additional copy requested at the same time.
Another method of searching for birth records would be to search for the same online using online databases. There are a number of database that could be used in this regard, and because they are so wide spread, they are easy to locate. They are even easier to use, and able to provide their information, which is the same as the information in government offices, in a faster and more efficient manner. In addition, some of them do not charge anything for the use of their databases.

- Download the application form from the website of the Oregon Health Authority
- Fill up the application form as accurately as possible.
- Make a copy of any government issued identification card with your signature visible. Attach the same to the application form.
- Have the application form notarized.
- Make a money order to correspond to the required fee in accordance with the required fees.
- Send all the requirements to the Oregon Health Authority for processing once completed.
