If this department can’t find the requested record, you will not receive a refund and will need to make requests to other departments to eventually find the record. Be sure to include all the information you have on the individual on the request form. You’ll be asked to provide a full name, date and place of death and date of birth (or approximate age) and, if possible, the social security number.
