In Oregon state, the primary state level agency that is tasked with keeping and maintaining birth records is the Oregon Health Authority, a branch of the executive branch of the Oregon state government. As part of their mandate, the Health Authority is also the only state level agency that is authorized to issue birth records, and the agency keeps records of birth, and other vital records, as far back as the year 1903. Records of birth occurring that date may be found at the local issuing authority of the county where the birth occurred or at the state archives.

To request for a birth record at the Oregon Health Authority, the searcher should first download the relevant form from the website of the Health Authority and to complete the same. Note that the form must be notarized because the request is for a certified copy of a birth record. Do not forget to include a copy of any government issued identification card with the signature of the searcher at the form. The next step would be to make a money order payable to the Health Authority to correspond to the required fee. The required fee is twenty dollars, plus fifteen dollars for every additional copy requested at the same time.
