Here, the searcher would be required to download the relevant form and fill up the same as accurately as possible before having the same notarized. The requirement of notarization is brought about by the fact that the record being requested for would be a certified copy. Once the form has been completed, the next step would be to make a money order to correspond to the required fee which is twelve dollars per record requested. The department would only accept money order to correspond to the required fee. Finally, the last requirement would be a copy of the front and back of any government issued identification card of the applicant where the signature of the applicant is clearly shown. Once all of this has been complied with, the requirements should be sent by mail to the department who would then conduct the search for the record.

Note that because most of these databases are not affiliated with the government, they could not really be used to establish identity, but rather, merely to confirm the existence of a person. There are a number of advantages that could be gleamed from using these online databases, such as the fact that while the data is the same, these online databases provide their information in a faster and more efficient manner.   • Visit the website of the department and download the relevant form
  • Fill up the form as accurately as possible and print the same. Alternatively, you could print it first then fill it up accurately.
  • Make a money order to correspond to the required fee and attach the same with the application form and other requirements.
  • Have a government issued identification card photocopied. Front and back must be at the same paper with your signature clearly visible and legible. Attach the same to the application form.
  • Send all the requirements to the department via mail.
